- Full contact information for corresponding author (particularly, if not submitted initially by author)
- List of contents (please include number of references, tables, figures)
- Manuscript (please refer to Manuscript Requirements) (Microsoft Word, double-spaced, 12-point Arial font, with page numbers, please)
- Tables (please include in manuscript file, placed at the end of document)
- Figures (please send figures as separate file(s). Adobe Illustrator [.eps], Adobe PDF [.pdf], TIFF [.tiff], or JPEG [.jpg] files, please)
- Journal Publishing Agreement
This form requires a signature from all authors and should be returned to the journal via fax or mail. The form is available for download here. If no form is received at this time, the form will be sent to authors with the letter acknowledging receipt of a manuscript.
